CHWP Application

To apply to participate in the College Health and Wellness Professional (CHWP) certification program, please submit the online form below. You will be contacted regarding acceptance into the program and then directed to pay the program fee.

Enrollment is accepted at any time, and, upon acceptance, coursework and examination are at your own pace.

1) Submit Application

The form below must be completed in one sitting. Please have your curriculum vitae in PDF or MS Word format available to attach at the end of the form.

Applications will be reviewed by the ACHA Leadership Institute Council. You can expect a response within 30 days.

2) Submit Program Fee

You may wait for an email confirming acceptance into the program before paying the program fee. The program fee is payable via credit online. If you cannot pay online, please contact Lisa Pitrone, lpitrone@acha.org.

ACHA individual members at institutional member schools = $300
ACHA individual members = $400
Nonmembers = $700

Cancellation/Refund Policy

  • If an applicant is not approved for acceptance into the program and has prepaid, the application fee will be refunded in full.
  • The program fee and certification renewal fees are otherwise nonrefundable for any reason.
