The ACHA Sexual Health Education and Clinical Care Coalition requests your completion of its annual Pap Test and STI Survey, now in its 20th calendar year! This clinically based survey collects institutional information about gynecological services, Pap test collection and evaluation, and testing for STIs for the year 2009 (January 1-December 31, 2009). Last year six short questions on “Contraception Services/Pregnancy Testing” were added.
Over the years the ACHA Pap Test and STI Survey has served as an important benchmarking tool regarding management strategies and data collection, as well as a means to assess educational efforts and compare health center services.

TIPS:
- We encourage you to print out a copy of the survey before completing it online to assist you in your data preparation and collection. To print, open the survey, print the page then scroll to the bottom and select “Go To Next Question.” You will need to print each of three “pages” of questions.
- When you reopen the survey to fill in your data, please select “resume from first page.” Similarly, if you begin entering data, pass the first page break, then need to exit the survey and return later, please select “resume” and the survey will take you to the page where you left off.
- Print a copy of your final submission. Please keep the copy of your submission for comparison purposes once the group results are circulated.
- You do not have to provide data for all of the sections, though we do encourage you to complete as many questions as possible. For example, even though you may not have complete Pap test data, you should still enter your STI data, even if those are the only questions you can answer.
Results of the survey will be emailed to participants in summer 2010, posted on the ACHA website, and ACHA members will be sent a link via a blast email later in the summer.
